Student Forms:
Listed below is the website link for parents/guardians to complete the following forms which are required for all MSM students.
PLEASE NOTE: The heading on the website page will say "Welcome to the Athletics Parent Portal"; however, the site also has forms required for ALL MSM students.

- Off-Campus Lunch Form (Only needs to be completed for seniors if you would like them to be able to go off-campus for lunch.)
Note: You must have your student's ID number to complete the online forms. If you do not have your student's ID number, please call the main office (405-631-8865).
Student Transportation Forms:
Student transportation forms are only needed for use to and from specific MSM events. Please consult with the director of your event, field trip or sport before using these forms.

Student Athlete Forms:
The forms listed below are required for all MSM student athletes.
Please note that every student athlete needs to complete, sign, and have a parent sign the following forms:
- Concussion Acknowledgment Form
- Sudden Cardiac Arrest Acknowledgment
- Permission to Transport
- Code of Conduct/Insurance Acknowledgement
Each MSM student athlete is also required to fill out a physical form.
Please print the physical form, complete both sides and return to MSM Athletics. A student is not allowed to participate (practice or play) until this physical form is on file with the school.
