Physical Form
Every MSM athlete, grades 9-12 is required to complete a physical before they participate in any Rocket sports.

According to the OSSAA:
No student shall be eligible to represent his/her school in athletics until there is on file a physical examination and parental consent certificate. Other forms may be utilized by the physician, physician’s assistant, or the advanced practice nurse, if the information contained is compliant with the information on the OSSAA form. A qualified physician, physician’s assistant, or an advanced practice nurse covered by professional liability insurance shall give the physical examinations.
Physical examinations are required for students each year. All physicals given for OSSAA participation must be given before the first day of practice in that student's particular sport. The physical will be valid from the date of the physical given until the next required physical – which would be before the first day of practice the following year. Parent(s) or guardian(s) must sign the parental consent form each year before the student participates in any organized athletic practice session including contest participation.
Mount St. Mary has set up a date for physicals. That date will be Wednesday, July 18 at the MSM Gym. We will be using the weight room and it will be first come first served. We will start at 5:30 so plan to arrive early or you may have to wait in line. The cost is $10 – cash or check – made payable to OCO.
Attached is the two-page OSSAA physical form. It will speed things up considerably if page one of the form is completed in advance. There will be other required MSM forms for each student athlete to complete. Those forms will be distributed by the coach of each sport at a later date.
If your student athlete is participating in a sport that may start practice before July 18 (Cross Country, Softball, Volleyball, or possibly Football), they will be REQUIRED to have on file a physical examination and parental/guardian consent signature.
If this date is not good for you, please feel free to have the physical exam done at another time and place. See above about using a different form.
